BEFORE YOUR APPOINTMENT:


  • We contact your primary care physician and/or specialists to obtain lab and test results.
  • We check your insurance to make sure your nutrition consultation is covered.
  • Once you have received your appointment, we ask that you give us 24 hours notice if you have to cancel.
  • Two days prior to your appointment you will receive a confirmation call/ text asking you to confirm your appointment.
  • Please click on the forms box to open the initial paperwork documents  
  • Bring the completed documents with you to your appointment.
  • We can also email or fax these forms to you as well.​
